For a Confirmation, please create a text field over your Notes Block and title it as 'Notes' in your configuration. I mean a Title with Notes, below the Title your block will be there. Just try this.....

In Runtime repository please check SRQM_NOTES component is used under ViewSet SRQM_INCIDENT_H/IncidentHeaderEF. that is SRQM_NOTES component should be called under Component usage

You can display multiple items for a single header using your model node.. For that you have to build your collection in that way, header record should repeat till item records end.
